Payments may be done online, thru bank deposit or walk in to our center. 

(Cash or check payable to Active Thinkers Child Development Center, Inc.)

Fill out Information Sheet 

This can be done online or by visiting our center.

Submit Requirements

•   PSA Birth Certificate   

•   3 pieces 2x2 photo

•  Medical Certificate issued by

    the child’s pediatrician
